A choroidal nevus is a flat, pigmented spot in the choroid, the vascular layer beneath the retina. It is essentially a freckle inside the eye, and like a freckle on the skin it is common and usually harmless.
Most are discovered incidentally during a routine dilated eye examination in someone with no symptoms at all. The purpose of specialist evaluation is not to treat the nevus but to document it carefully and establish whether it shows any features that warrant closer surveillance.
Why a benign finding is still worth following
The overwhelming majority of choroidal nevi remain unchanged for life and never affect vision. A small proportion, however, can grow, and rarely a lesion that appears to be a nevus is in fact a small choroidal melanoma.
There is no single test that settles this at a first visit. What distinguishes a stable nevus from a growing lesion is, in most cases, observation over time — which is why the standard approach is careful baseline documentation followed by scheduled re-imaging rather than immediate intervention.
This is surveillance in the same sense that a dermatologist photographs and re-checks a mole. It is a precaution, not an indication that something is wrong.
Features that prompt closer attention
Certain characteristics are associated with a higher likelihood of growth and lead to shorter monitoring intervals:
- Increased thickness of the lesion
- Subretinal fluid overlying or adjacent to it
- Orange pigment on the surface
- Symptoms attributable to the lesion, such as blurring, distortion or flashes
- Proximity to the optic nerve
- Absence of drusen or other signs of long-standing stability
A nevus with none of these features and a stable appearance over successive visits is generally followed at longer intervals. Your physician will explain which, if any, apply to your lesion and what interval is appropriate.
The imaging used to document it
Baseline documentation typically combines several methods, each showing something the others do not:
- Fundus photography records color, size and border, providing the reference image against which later photographs are compared.
- Optical coherence tomography (OCT) shows the retinal layers above the lesion and detects subretinal fluid that is not visible on examination.
- Fundus autofluorescence highlights orange pigment and areas of cellular stress.
- Ocular ultrasound (B-scan) measures thickness, which is among the most useful measurements for tracking change.
These are non-invasive and painless, and the same set is generally repeated at follow-up so that measurements are directly comparable.
What monitoring looks like in practice
After a baseline visit, patients are usually asked to return at an interval determined by the appearance of the lesion — commonly within several months initially, then at longer intervals if it proves stable. Many patients settle into annual imaging.
Between visits, no restrictions or precautions are required, and a nevus does not need to be protected or treated. Sunglasses are sensible for general eye health but do not affect a choroidal nevus.
Contact the office sooner if you develop new blurring, distortion, flashes, floaters or a shadow in the vision of that eye. Those are not expected with a stable nevus, and they are worth examining rather than waiting for the scheduled appointment.
If a lesion does show change
Documented growth or the development of concerning features prompts referral to an ocular oncology service for further evaluation. That referral is a step toward a definitive assessment, not a diagnosis in itself.
The value of the monitoring schedule is precisely that it makes change detectable while lesions are small. This is the reason a benign finding is followed at all.
