Diabetic retinopathy is damage to the small blood vessels of the retina caused by long-term elevated blood sugar. It is among the most common causes of vision loss in working-age adults, and its most important characteristic is that it usually causes no symptoms until it is well established.
That silence is the reason a dilated retinal examination is recommended for people with diabetes on a regular schedule regardless of how good their vision feels.
What patients typically notice — and when
Early diabetic retinopathy is generally asymptomatic. Vision is normal, reading is normal, and nothing signals that the retina is changing. Damage detectable on examination is often present well before anything is noticeable.
When symptoms do appear, they may include:
- Blurred or fluctuating vision, sometimes varying with blood sugar levels
- Dark spots, floaters or streaks drifting through the vision
- A dark or empty area in the center of vision
- Difficulty with night vision or with color perception
- Sudden, substantial vision loss in one eye, which may indicate bleeding into the vitreous
Because symptoms arrive late, they should not be used as the trigger for an eye examination.
Why elevated blood sugar affects the retina
The retina is supplied by an unusually fine network of blood vessels. Sustained high blood glucose weakens the walls of these vessels, which can then leak fluid and blood into surrounding retinal tissue or become blocked entirely.
In the earlier, non-proliferative stage, vessels leak and small areas of swelling and hemorrhage appear. If leakage occurs at the macula the result is diabetic macular edema, which is the most common way diabetic retinopathy affects central vision.
In the more advanced proliferative stage, areas of the retina become deprived of blood supply and the eye responds by growing fragile new vessels. These vessels bleed easily and can form scar tissue that pulls on the retina, leading to bleeding into the vitreous or a tractional retinal detachment.
Duration of diabetes, degree of blood sugar control, blood pressure, cholesterol, kidney disease and pregnancy all influence how quickly this progresses.
Getting evaluated
A diabetic eye examination is performed with the pupils dilated so the entire retina can be inspected, not just the central portion. Optical coherence tomography measures retinal thickness and detects macular swelling that is not visible on examination alone.
Where leakage or abnormal vessel growth is suspected, fluorescein angiography maps retinal blood flow and shows precisely where vessels are leaking or where the retina has lost its blood supply. Widefield imaging can capture peripheral retina that a standard photograph would miss.
Findings are graded by stage, which is what determines both treatment and the interval before the next examination.
Treatment options
Treatment is directed at the specific problem identified.
- Anti-VEGF injections are commonly used for diabetic macular edema and for proliferative disease, with the goal of reducing leakage and abnormal vessel growth.
- Corticosteroid treatment delivered into the eye is an option for some patients with persistent macular edema.
- Laser photocoagulation may be used to treat leaking vessels or, in panretinal form, to reduce the stimulus for abnormal vessel growth in proliferative disease.
- Vitrectomy surgery may be considered when there is bleeding into the vitreous that does not clear, or when scar tissue is pulling on the retina.
None of these replaces medical management of diabetes itself. Blood glucose, blood pressure and cholesterol control, coordinated with your primary care physician or endocrinologist, remain fundamental to how the retina does over time.
Living with the diagnosis
A diagnosis of diabetic retinopathy is not in itself a prediction of vision loss. Many patients with early disease remain stable for years, particularly where systemic control is good and examinations happen on schedule.
What matters most in practice is not missing appointments. Because the condition can advance without symptoms, an interval that feels unnecessary is often exactly the one that catches a change early enough to act on.
Report any sudden increase in floaters, new dark areas, or an abrupt drop in vision without waiting for the next scheduled visit.
